Applications submitted in error or not approved for eligibility are non-refundable. Please be sure to follow the instructions below, step-by-step, to avoid submission of an application that is not eligible for processing.

After submission of your application, you can verify the status of the application on the Nurse Portal Dashboard when logged into your account. Please allow at least 2 weeks for the NM BON to receive any documents from third parties before the application can be processed.

Requirements for licensure as a Lactation Care Provider per Regulation 16.12.11.11 NMAC:

  • Be at least 18 years of age.
  • Hold an approved certification as a lactation provider
  • Complete the required application and pay fee of $60
  • Completion of a federal criminal background check

It is the responsibility of the applicant to ensure that the application requirements are met and documents are received by the NM BON prior to the expiration of the application.

Certification

You will be required to upload your certification at the time of application. Additionally you will be required to obtain first party verification of your valid certification. The verification must come directly to the NM BON from the accrediting agency.

  • International Board Certified Lactation Consultant Verification Form
  • Certified Lactation Counselor Verification Form

Criminal Background Check

Criminal Background Check required for Lactation Care Provider applicants: Complete processing of fingerprints can take up to two weeks and in some cases up to 90 days, Gemalto will release the results to the NM BON when they are made available. Please keep in mind, NM BON will only retain the results for 30 days from the time they are made available. You would want to do your fingerprints no earlier than two weeks before you plan on submitting your application.

In-state Applicants ONLY

  1. Register online or by telephone 1-877-99NMAPS (1-877-996-6277).
  2. Pay for your criminal background check ($44).
  3. Choose a fingerprint location
  4. Travel to chosen fingerprint location and pay for your criminal background check (if not already paid online) and any additional fees required by the fingerprint location.

Out-of-State Applicants ONLY

  1. See NM Applicant Processing Service Handout
  2. Obtain two fingerprint cards FD-258 or go to Online Services Finger Print Cards Request.
  3. Register online or by telephone 1-877-99NMAPS (1-877-996-6277).
  4. Payment ($44) may be made online at this time or sent with the fingerprint cards when completed.
  5. Get fingerprinted.
  6. Write the registration ID on the back of the finger print cards
  7. Mail fingerprint cards to; GEMALTO COGENT APS DEPARTMENT #165 2964 Bradley Street Pasadena, CA 91107
