Requestors of Nursing Lists
The information on the list includes:
- Name
- License Number
- License First Issued Date
- License Last Issued Date
- License Expiration Date
- Mailing Address
- Email address
- Primary Specialty
- Education Basis for License
- Prescriptive Authority Status
Data lists are updated at the beginning of each month. NMBON is unable to provide receipts for Datalist purchases. Data lists are provided in Microsoft Excel.XLSX format and only include active licenses.
PLEASE NOTE: Please fill out the form below, an invoice will be emailed to you indicating the cost of the requested nursing list. Please allow one week processing time.
